Life Challenge Transition Statement

Transition Outline

Creating a dynamic transition plan is not easy. After much thought and prayer on student transition, we have identified three areas of transition that we feel will have a significant impact on the lives God has entrusted to us. The areas to be addressed are:

  1. Transition In – demonstrating an active role in assessing students needs and concerns as they enter our program…early involvement and planning.
  2. Transition Thru – a continuous active support plan that addresses ongoing concerns, issues and post-LC objectives.
  3. Transition Out – providing a personalized transition plan that identifies the multiple areas of infusion upon graduation.

As we develop and implement the necessary components for each transition phase, it is important for us to understand what will best serve the students later on in life as it relates to their personal relationship with Jesus Christ. To that end, it should be noted that our primary emphasis will be focused on strengthening their relationship with God and a community of believers… the local church. Employment and educational opportunities, as well as housing concerns will be considered post-secondary objectives.

Transition-In Components / 1 - 60 Days

  • Program welcome entrance meeting---Department Heads.
  • Provide selected senior students for peer support & program outline follow-up…living syllabus.
  • Transition-In interview…assessment / local church inquiry
  • Thursday Night Small Group / Lower Growth (0 – 30 days)
  • Encourage in-house prayer participation
  • Encourage outreach participation (after 30 days)
  • Local church inquiry follow-up
  • Initiate relationship development with continuous follow-up
  • Schedule weekly CBWA ( counseling by walking around)

Transition-Thru Components / 2 – 10 Months

  • Continue relationship development
  • Continue local church inquiry follow-up
  • Continue monthly assessment
  • Encourage outreach & in-house prayer participation
  • Bi-weekly CBWA
  • Identify post-LC objectives

Transition-Out Components / 10 – 12 Months

  • Design personalized transition plan
  • Evaluate post-LC objectives
  • Identify post-secondary objectives
  • Continue relationship development
  • Continue local church follow-up
  • Continue encouragement for outreach & in-house prayer participation
  • Continue monthly assessment
  • Continue bi-weekly CBWA
  • Assist the student & family to clarify desirable outcomes and encourage their active participation in transition planning
  • Graduation follow-up

In closing, as we implement and continue to define the appropriate student transition plan as it relates to Life Challenge objectives, we are confident our aim to better serve each student will glorify God.
